How to start
- Open your survey link — the one shared in your facility’s group. It opens in your web browser; there’s nothing to download or install.
- (Optional) For easier access, tap your browser’s menu → “Add to Home Screen” to save it like an app. You can also just use the link.
- Read the consent screen. It explains what the survey is for and what’s collected. The questionnaire unlocks only after you agree to continue.
While you answer
- Answer at your own pace — there’s no timer per question.
- The form only shows questions that apply to you and will gently flag an answer that looks off, so it’s easy to get right.
- Your answers save automatically. You can stop and come back later — on the same device and browser — and pick up where you left off.
- A weak connection is fine: the form saves your answers and submits when you’re back online.
Finishing
- Answer through to the end and tap Submit.
- You’ll see a clear “Submitted” confirmation once it’s safely received — that means you’re done.
- Please complete it within the window your facility gives you (usually a few days of first opening the link).
No “Submitted” confirmation? You’re not done yet —
re-open the link on the same device to resume and
submit.
Your privacy
- Your responses are confidential and used only for the UHC Survey.
- ASPSI is registered with the National Privacy Commission (PIC-000-358-2021) and handles your data under the Data Privacy Act (RA 10173).
- The consent screen tells you exactly what is collected (including a submission timestamp) before you agree.
- Taking part is voluntary — it does not affect your employment or your work in any way.

| Any other question | Contact your facility’s survey focal person. |
Paper option: if you can’t complete it online, ask
your facility focal person for a paper copy — hand it back before
the survey team leaves, and ASPSI will encode it for you.
