Universal Health Care Survey · Year 2
Help & documentation
How to sign in, monitor fieldwork in CSWeb, and collect and sync data on a tablet. For authorized project personnel — enumerators, supervisors, and DOH monitors.
Step-by-step guides
Full walkthroughs for the people who run and take the surveys:
- CAPI Manual — the complete tablet & application guide The full how-to in one place (17 sections): signing in, getting assignments, GPS, entering and reviewing answers, completing and syncing, the Supervisor & Enumerator Hub, troubleshooting, and data security — illustrated with real app screenshots and flow diagrams. Start here for the whole picture; the focused guides below cover single tasks.
- CSEntry on your tablet — enumerator guide For field enumerators (F1 · F3 · F4): install, connect, download the survey, run an interview, and sync.
- Supervisor & Enumerator Hub — sign-in & role menu The recommended way to work in the field: one role-based sign-in that opens a menu for your role and launches the F1/F3/F4 surveys — and lets supervisors hand out assignments, collect interviews over Bluetooth, and relay them to the server. The direct way of adding each survey still works unchanged.
- Healthcare Worker survey — how to complete it For healthcare workers (F2, self-administered): opening the link, answering, finishing, and privacy.
- F1 Facility Head — paper ↔ CAPI comparison All 10 sections (Q1–Q166): every question beside its real CSEntry screen, with notes on skip logic, validations, and where the tablet differs. The check-box screen is still being captured.
- F3 Patient — paper ↔ CAPI comparison All 12 sections (A–L): every question beside the matching CSEntry control, with appearance and validation notes. The check-box screen is still being captured.
- F4 Household — paper ↔ CAPI comparison All 17 sections (A–Q), including the household roster and expenditure grids: every question beside the matching CSEntry control. The check-box and grid screens are still being captured.
- F2 Healthcare Worker — paper ↔ web (PWA) comparison (complete) All sections of the printed HCW questionnaire beside the real self-administered web (PWA) screens — fully captured from the live web app.
- Sync Dashboard — synced cases at a glance For supervisors/monitors: charts of uploaded F1/F3/F4 cases — totals plus breakdowns by region, facility, ownership, patient type, sex, and result of visit. Refreshes automatically as cases sync.
- Map Report — synced cases on a map For supervisors/monitors: each F1/F3/F4 case plotted at its captured GPS point, coloured by status, with filters for instrument, region, status, visit date, and data quality. F3 shows both the facility and the patient-home location (toggle each, or connect the pair). Field-QA flags surface low-accuracy fixes, duplicate respondent locations, displacement (a home far from its facility, or a facility point off its cluster), and wrong-area cases (coordinates that fall outside the declared province). Cases with no GPS fix are counted but not plotted. Refreshes automatically as cases sync.
The two systems
The platform has two separate systems, one per group of instruments. Each has its own sign-in.
- CSWeb — the server for the Facility Head (F1), Patient (F3), and Household (F4) surveys. Enumerators collect these on Android tablets in CSEntry; supervisors and monitors watch the data arrive here.
- HCW Admin Portal — administration for the self-administered Healthcare Worker (F2) survey, which respondents fill in on the web.
Signing in
From the home page, choose your system. Each system uses separate credentials — a CSWeb account does not work on the HCW portal, and vice versa.
-
CSWeb: open
csweb.asiansocial.organd sign in with the username and password issued to you. - Enumerators use the same CSWeb account inside the CSEntry app on the tablet (see “Collecting on a tablet”).
Using CSWeb (monitoring)
After signing in to CSWeb, the left menu has the tabs you’ll use to follow fieldwork:
- Data Browse the cases that have synced from the tablets, per instrument. Open a case to see its answers.
- Sync Report A running count of uploaded cases by facility. Pick an instrument from the sub-menu; the table lists each questionnaire number with its facility name and total, and “View case” opens the record.
- Map Report Plots cases that captured a GPS fix on a map of the Philippines — useful for confirming coverage by area.
- Apps The CAPI applications that enumerators download to their tablets. New questionnaire versions are published here.
Users, Roles, and Settings are for administrators — managing accounts and access. Most field and monitoring staff won’t need them.
Collecting on a tablet (enumerators)
Enumerators collect the F1/F3/F4 surveys on Android tablets in
CSEntry (free, from Google Play — publisher
U.S. Census Bureau). In short: install CSEntry, add your
survey from the server
(https://csweb.asiansocial.org/csweb/api) with your own
CSWeb account, then run interviews — cases save on the device, so you
can work offline and sync later.
For the full walkthrough, follow the step-by-step guides:
- Supervisor & Enumerator Hub — the recommended way: one role-based sign-in (install LoginApp), a menu for your role, and Bluetooth assignment/collection.
- CSEntry enumerator guide — installing, connecting, downloading the survey, running an interview, and keeping the app current.
- CAPI Manual — the complete reference for everything above and more.
Syncing data
Syncing uploads your new and changed cases to the server — do it whenever you have a stable connection, at least once at the end of each field day. From your survey’s case list, tap Synchronize (the circular-arrows icon) and wait for the “Successfully synced” confirmation. Full steps are in the enumerator guide; the supervisor Collect → Relay path (the no-signal safety net) is in the hub guide.
Your supervisor or monitor can then confirm the case appears in CSWeb → Sync Report, and — if the GPS got a real outdoor fix — that a pin shows on the Map. An indoor case may have a blank GPS; that’s expected.
Troubleshooting
-
Can’t connect or download: check your internet,
and type the server address exactly, including
https://and/csweb/api. - Login fails: re-type the username in lowercase with no spaces, and confirm the password.
- Screens look old (one question per screen): you’re on an older version — remove the app and add it again from CSWeb (or use Add Application → Update). The “⋮ → Update Installed Applications” shortcut can miss new versions.
- GPS won’t read: move near a window or step outside, and make sure Location is allowed and switched on. Indoors it may time out and stay blank — note it and continue.
- Camera won’t open: allow Camera in Settings → Apps → CSEntry → Permissions.
- Sync fails: confirm internet, re-enter your account, and retry. If it still fails, capture a screenshot of the error for your coordinator.
Support
For account credentials, field issues, or anything not covered here, contact your survey coordinator or team supervisor. When reporting a problem, a screenshot or photo of the screen and the questionnaire number involved helps us resolve it faster.
